The consent form can be filled out with Adobe Acrobat or you can print and fill it out.
Submit the Consent Form

You must fill in all information, and prescription information, including RX BIN and PCN numbers.

You can bring the form with you on the day of your vaccine.

Currently, the Pfizer and Moderna booster vaccines are authorized for ages 12 and older. Pfizer and Moderna boosters are eligible to anyone 2 months after their last dose"

Where can I get a consent form? Is it available online?

Our consent form is now available online for the Pfizer, Moderna and J+J vaccine. You must fill out the form and it bring with you on the day of your vaccine.

Do I really need to fill out the entire application, including insurance information and demographic information?

You must fill out the entire form, front, and back. We need all of your information filled in for our records and for the state's records. You must include all prescription information including RXBIN and PCN numbers.

I can't edit the PDF of the online application form. What do I do?

You must have a program like Adobe Acrobat Reader to directly edit the document. If you do not, you need to print out the form, fill it out, scan it and email it in. We also have a limited number of printed applications in-store.
